PrimaCare Community Family Health Team encompasses an Executive Director, Interdisciplinary Health Providers (IHPs) and Administrative Staff. Our staff work with and provide support to nine (9) Family Physicians who collectively form a Family Health Organization (FHO). In addition, each Family Physician independently employs and manages their own administrative staff.
Our Physicians are the key members of the Family Health Team. Our Physicians provide the majority of your medical care. They work with our IHP professionals to offer additional care and services.
Our Nurse Practitioners (NP) are registered nurses with additional education, training, and nursing experience that prepares them for advanced nursing practice. They provide a wide range of health services such as perform physical exams, order and interpret tests, diagnose and treat diseases and other health conditions, as well as prescribe medication. Nurse Practitioners work in conjunction with Physicians and collaboratively with other nursing colleagues and health-care providers, or they may work independently.
Our Registered Nurses (RN) work directly with the Primary Care Provider and collaboratively with other health professionals within the Family Health Team to provide health assessments, treatment, disease management, and education.
Our Registered Social Worker (RSW) provides the psychosocial perspective to primary care and focuses on the assessment, treatment, and prevention of mental illness, emotional, and other behavioral disturbances. This includes individual and group counselling, links to other community resources, education on mental health issues, and promoting mental health advocacy.
Our Child and Youth Mental Health Counsellor (CYCP) provides family centred care as the impact of the family unit and/or caregivers on a child’s learning and development are crucial. The CYCP works with families to provide education and support in addition to the skills and interventions that are taught to the youth themselves. The family will be an active part of the process for our younger patients as will school and any other major influences.
Our Psychiatrist is a medical doctor who specializes in the diagnosis, treatment, and prevention of mental, emotional, and behavioural disorders. They are trained to evaluate a person’s mental health through interviews, observations, and psychological tests. In addition to providing therapy and counselling, psychiatrists can also prescribe medication to help manage mental health conditions. They work with individuals of all ages and backgrounds, including children, adolescents, adults, and older adults. The goal of a psychiatrist is to help their patients improve their mental well-being and overall quality of life.
Our Registered Dietitians (RD) are health professionals who are experts in nutrition and the human diet. Dietitians assess, counsel, and support their patients to make dietary changes to promote and improve health, as well as prevent and treat chronic illness and disease. They help shape the dietary choices of their patients by using a combination of scientific research, nutrition, behavioural science, and social science.
Our Pharmacists are highly skilled health professionals who use their expertise in medication management and drug therapy to serve as a bridge between patients, Physicians, and other healthcare providers, ensuring safe and effective medication use and seamless care. In addition, pharmacists are involved in providing clinical services such as smoking cessation, diabetes management, blood pressure monitoring and management.
Our Chiropodist is a primary health care provider, described as a foot specialist, who can assess and provide medical treatment of nail and skin concerns, including nail care, corns and calluses, perform procedures, assess and treat chronic and acute conditions affecting foot and lower limb function, as well as diabetic foot care and assessment.
