COMPASSION AWARD 2024 – NOMINATION DEADLINE

AVAILABLE TO ALL COMMUNITY MEMBERS!

This is the first year that PrimaCare Community FHT will be presenting this award during Mental Health Week 2024. Established in 2024 by Jennifer Smith, Mental Health Week Committee Lead and Administrative Assistant of PrimaCare Cmmunity FHT.  Open to anyone ages 16yrs and older who consistently personifies genuine compassion and kindness with others and practices self-compassion, and who contributes to a positive and inclusive atmosphere.

All nominations must be accompanied by a completed Nomination Form (https://www.pccfht.ca/news-2/), and submitted by email to Jennifer Smith jsmith@pccfht.ca or in person at PrimaCare FHT 25 Curtis Avenue North, Suite 201 Paris ON by Monday April 29, 2024.
