At PrimaCare Community Family Health Team (PCCFHT), we offer a variety of services and programs to meet the needs of our patients. In addition to the care that you receive from your Family Physician, PCCFHT also has other health care providers available to support your needs. These programs aim to increase the quality of your health, by either helping to manage current conditions or assisting in preventing future health problems.
The After Hours Clinic is for patients of the Family Health Team who need to be seen the same day and cannot be booked in regular office hours. Please contact your Family Physician’s office first to inquire about an appointment and your Family Physician will then refer to the After Hours Clinic if appropriate.
Please note: This service is not intended for routine office appointments, referrals, prescription renewals, test requisitions, acute and/or chronic conditions or emergencies.
Monday to Friday: Starting at 5 pm
Saturday: 9 am-12 pm
During the clinic, a Registered Nurse can assist patients with concerns such as ear syringing, B12 and prolia injections, suture and staple removal, newborn baby check-ups, women’s exams, STI testing, birth control counselling and memory testing!

We offer free blood pressure measurement and monitoring sessions. The Blood Pressure clinic is used to promote cardiovascular health in our community and to raise awareness about the importance of blood pressure monitoring.
The clinic is open to the entire community, so you do not need to be a patient of PCCFHT to access this service. No referral or appointment is required and a copy of your results will be provided to you.
The clinic is located at 25 Curtis Avenue North, Paris on the first floor in the community room.

A Chiropodist is a primary health care provider, described as a foot specialist, who can assess and provide medical treatment of nail and skin concerns, including nail care, corns and calluses to promote healthy, pain-free feet. Foot issues can be a very common in patients with diabetes and can lead to serious complications.
Please note: The Chiropodist only accepts referrals for Diabetic Foot Assessments and ABI Assessments from your Family Physicians office. All other appointments must be booked as a same-day appointment. Please contact the Family Health Team Reception starting at 8:30 am to request an appointment.
The RN Foot Clinic offers pre-booked appointments.
The Registered Nurses are certified to provide basic toenail care such as trimming, filing and assess the condition of your feet while checking for any inflammation, bruising, cuts, or blisters. They are able to provide valuable services that benefit those who have foot conditions related to diabetes and other illnesses. The RN will refer you to the Chiropodist for any concerns outside their scope of practice.

Registered Dietitians can assist with a broad range of conditions across the lifespan, including but not limited to: pre-diabetes, type 1 and type 2 diabetes, dyslipidemia, hypertension, irritable bowel syndrome, crohn’s disease, colitis, osteoporosis, infant/toddler/preschooler/school-aged child nutrition, disordered eating, pregnancy nutrition, weight loss or weight gain, chronic kidney disease stages 1-3, food insecurity (low-income households) and general nutrition counseling.

The INR clinic is for patients who are on Coumadin (also known as Warfarin) and are at an increased risk of forming a blood clot or who have recently experienced a blood clot. While these patients are on Coumadin they will be required to have their blood checked on a frequent basis. The INR clinic is run by our Pharmacists, Natalie and Lindsay.

The Pharmacists work with our FHT patients and provide health-related consultations, specialty health care services and advice. The goal is to help improve the quality of life for our patients. At PrimaCare we believe that these services play a major role in increasing access to primary care and reducing costs to the health care budget.

Smoking Treatment for Ontario Patients (STOP) Program is designed to support you in your smoking cessation journey by providing behavioural counselling and up to 26 weeks of Nicotine Replacement Therapy (NRT) free of charge.

Social Workers assist clients in dealing with life and mental health challenges by delivering counselling, community services, and social support programs.
PCCFHT counselling Services provide professional counselling to individuals, couples, families and groups in a safe, confidential and supportive environment. Therapeutic counselling is a specialized short-term service that addresses the psychosocial, emotional, behavioural, relational and mental health needs of PCCFHT patients. When available, psychoeducational groups are open to the entire community.

PrimaCare offers adult psychiatry services for patients of the Family Health Team.
Patients 17 and older can be referred by their Family Physician, Physician Assistant or Nurse Practitioner for diagnosing and monitoring of various mental health conditions such as ADHD, anxiety, depression, post-partum depression, gender identity disorders, mood and personality disorders.
We have a Registered Nurse who is certified to conduct and interpret spirometry testing. Spirometry is a common test used to assess how well your lungs work by measuring how much air you inhale, how much you exhale and how quickly you exhale. This test is used to diagnose breathing problems like asthma, chronic obstructive pulmonary disease (COPD) and other conditions that affect breathing.
Our Respiratory Therapist (RT) can assess the cardio-respiratory systems of patients, perform and interpret diagnostic breathing tests, and develop and implement respiratory therapy care plans for clients. The RT also monitors and evaluate patient progress especially for patients with chronic and severe breathing problems and conditions.
