After Hours Clinic
The After Hours Clinic is for patients of the Family Health Team who need to be seen the same day and cannot be booked in regular office hours. You need to call your Family Physician’s office first and your Family Physician will then refer to the After Hours Clinic if appropriate.
Note: This service is not for routine office appointments or urgent issues.

Blood Pressure Clinic
At PrimaCare we offer free blood pressure measurement and monitoring sessions. The Blood Pressure clinic is used to promote cardiovascular health in our community and to raise awareness about the importance of blood pressure monitoring. A copy of your results will be given to you and no referral is needed.
Trained volunteers will help you:
Complete a checklist about risk factors for heart disease and stroke
Address modifiable risk factors
Wednesday: 1:30PM – 3:30PM
Thursday: 9:30AM – 11:30AM
A Chiropodist assesses and treats foot conditions. Foot problems are very common in people with diabetes and can lead to serious complications. Referral needed from your Family Physician’s office.
Some of the conditions a patient may be referred for include:
Corns, callouses, warts
Wednesday and Thursday: 9:00AM – 3:30PM
A Registered Dietitian (RD) is a health professional who can provide advice on nutrition, food and healthy eating choices to help you achieve and maintain a healthy lifestyle. Whether you are trying to manage a medical condition with a special diet, control your weight, feed your family, or interpret nutrition facts on food labels, a Registered Dietitian can help. Registered Dietitians are your trusted experts in nutrition. Referral needed from your Family Physician’s office.

International Normalized Ratio (INR)
For patients who are on Coumadin (also known as Warfarin). These patients are at an increased risk of forming a blood clot or for patients who have experienced a blood clot recently. While these patients are on Coumadin they will be required to have their blood checked on a frequent basis. Referral needed from your Family Physician’s office.

Monday to Thursday: 9:00AM – 12:00PM
Friday: 1:00PM – 4:00PM.
Never Too Late Seniors Fitness Program
Physical activity at any age is important and has many benefits. Never Too Late is a popular Kinesiologist- supervised exercise program offered at the Willett Hospital. It is a 1 hour class and programs are tailored to individual needs. Referral from Family Physician’s Office preferred but not necessary.
Cost: $35 per month for 2 times per week or $50 per month 3 times per week
Monday and Wednesday: 9:30AM, 11:00AM, and 12:30PM

Ontario Telemedicine Network (OTN)
Telemedicine uses video cameras, Store Forward software and monitors to connect you to specialists who are not located near you, reducing the need to travel to receive care. You will be able to see, hear and talk to a doctor or other health care professional. A telemedicine appointment is just like a regular appointment; only the specialist you will be seeing and speaking with is on a monitor. Referral needed from your Family Physician’s office.

Assist clients in dealing with mental health challenges by delivering counseling, community services and social support programs. Referral needed from your Family Physician’s office.
This is achieved through:
Educating others on the impact of social and environmental factors
Developing, promoting, and delivering programs in collaboration with other professions
PCCFHT Counselling Services provide professional counselling to individuals, couples, families and groups in a safe, confidential and supportive environment. Therapeutic counselling is a specialized short term service that addresses the psychosocial, emotional, behavioural, relational and mental health needs of individuals, couples and families and is open for PCCFHT patients. Psychoeducational groups are open for both PCCFHT patients and the community.
